Consular officer in Odesa and Lviv on September 23, 24

A Consular Officer will be on a trip to Odesa on Tuesday, September 23, 2008 and will be available to provide Voting assistance and a range of Consular services in the "Window on America" center, located in Odesa Scientific Library at # 5, Katerynynska Ploscha str. from 3:00 p.m. till 4:30 p.m.
A Consular Officer will be on a trip to Lviv on Wednesday, September 24, 2008 and will be available to provide Voting assistance and a range of Consular services in the "Window on America" center, located in Lviv Oblast Library for Youth at # 9, Ploscha Rynok from 10:00 a.m. till 11:30 a.m.

The range of Consular services includes:
* Voting Assistance: To register to vote and/or apply for an absentee ballot, you can use the Federal Post Card Application http://www.fvap.gov/overseas-citizens/request-registration-absentee-ballot/index.html The on-line version of the application is accepted by all states and territories except American Samoa and Guam. You may alternatively request an Absentee Ballot through the U.S. Embassy by filling out a standart form that the Consular Officer will be able provide you with and mail it for you through the Embassy diplomatic pouch.
* Notary services (affidavits, powers of attorney, acknowledgements etc.): You may either prepare the document in advance or use our standard blank form on the spot. Please do not sign the document in advance. You will have to sign it under oath in front of the Consular Officer. The fee is USD 30 cash for the first seal and USD 20 for each additional seal provided at the same time in connection with the same transaction.
* Passport application (first child passport, first adult passport, regular renewal or replacement of a lost/stolen passport): While we cannot issue a passport on the spot, we can accept passport applications and send them to the U.S. National Passport Center. All required forms can be filled out on the spot or you can download them in advance from the website of the Department of State at http://travel.state.gov/passport/forms/forms_847.html The following items are necessary for your application:
* For a child’s first passport, regular renewal or lost/stolen passport replacement - USD 85 cash, originals, photocopies and English translations (if needed) of the child’s birth certificate, parents’ marriage certificate, both parents’ passports, and two passport-size photos of the child (2”x 2”). Additionally, in cases of lost/stolen passports we will require a police report. In all cases, the presence of the child is a must. Please note that in almost all cases the consent of both parents is required if the child is under 16 years of age. Please contact the American Citizens Services Unit using the information below if both parents will be unable to appear in person to sign the application.
* For the first adult passport - USD 100, an original and a photocopy of the expiring or expired passport, and two passport-size photos (2”x 2”)
* For regular renewal of an adult passport - USD 75, an original and a photocopy of the expiring or expired U.S. passport, and two passport-size photos (2”x 2”)
* For replacement of an adult lost/stolen passport - USD 100, originals and copies of photo IDs, a copy of the lost/stolen passport (if available), and a police statement.
