As specified by a new Adoption Decree (#1377), which became effective on January 1, 2004, the National Adoption Center (NAC) of Ukraine now requires all prospective adoptive parents to use a standard medical form, a copy of which can be downloaded here. Please note that when preparing adoption dossiers for registration as candidates for adoption in Ukraine, you must prepare the required medical certificates using only this special form.
Despite the many signature lines on the form, the NAC has confirmed that prospective adoptive parents do not have to go to several specialists to complete the medical examination. The examination can be performed by one physician - and the form can be certified by that doctor - so long as all information indicated on the form is complete.
The Center also informed the Embassy that all prospective adoptive parents whose adoption dossiers were either submitted or already registered at the NAC on or after January 1, 2004, are required to resubmit their medical certificates according to the new requirement.
